What type of condition would indicate a Need for Home Health Care?
Patients are typically referred to home health services by a physician following a change in “functional status.” Functional status is measured by one’s ability to perform activities of daily living, such as walking or transferring.
Also, patients that have new medications, new diagnosis and/or if they require health education regarding their own health will need home health care. A referral may be made after experiencing a significant medical event or change; including, but are not limited to:
- Neurological incidents
- Wound Care
- Cardiovascular disease
- Orthopedic events
- Generalized weakness
